- National Identity Card of claimant
- Birth Certificate of claimant/children
- Certificate of Attendance for children attending school
- Bank Account Number
- Proof of address
- Rent Book if any
- Payslip/ Memo from employer if any
Application may be registered at the Social Security Office nearest to the applicant’s locality.
