1. To beneficiaries of Basic Pensions who would have qualified for Social Aid in absence of basic pension; to Social Aid beneficiaries, beneficiaries of Unemployment Hardship Relief and to beneficiaries of SRM.
2. Subject to monthly Household Income not exceeding Rs 35,000 (including income of ascendants and unmarried children)

1. National Identity Card of claimant/Proxy/spouse
2. Birth Certificate of claimant/ spouse
3. Marriage Certificate
4. Quotation from approved supplier
5. Payslips of working members of household, if any
6. Documentary evidence of income from other sources
7. Prescription
