To be in receipt of Basic Invalidity Pension or Basic Retirement Pension.  

1. Bedridden.
2. Having an incapacity of 60% or more and needing constant care and attention of another person.
3. Same to be certified by a Medical Board or Medical Officer of the Ministry.

1. Birth Certificate.
2. Marriage Certificate, if applicable.
3. National Identity Card.
4. Medical Certificate.
5. National Identity card of Proxy.
6. Bank account number.  If joint account, copy of NIC of joint holder to be produced.

Application may be registered at the Social Security Office nearest to the applicant’s locality.
