3. Damage to natural teeth, clothing, spectacles or any artificial aid being worn or used at the time of the industrial accident
4. Medical, Surgical attendances and other expenses rendered necessary as a result of the industrial accident

2. National Identity Card.
3. Medical Certificates/Evidence; and
4. Bank Name and Bank Account Number.In the case of a joint account, the National Identity Card(s) of Joint account holder(s) should be produced.
