to be undergone by the beneficiary and the approximate duration of the treatment; or\u00a0<\/li><\/ol><p>\u00a0 \u00a0 \u00a0 \u00a0 \u00a0 \u00a0A medical certificate from the health institution abroad that the beneficiary will undergo, or has undergone, medical treatment and the duration of the treatment.<\/p><ol start=\"3\"><li>A copy of National Identity Card (NIC)<\/li><\/ol><ol start=\"4\"><li>A copy of Passport details (both local and foreign)<\/li><\/ol><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<div class=\"elementor-accordion-item\">\n\t\t\t\t\t<div
