Payable to an employee or self-employed who has suffered an industrial injury resulting in a disablement.<\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t

\n\t\t\t\t\t\t\t\t\t\t\t\t\t\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/i><\/span>\n\t\t\t\t\t\t\t\t<\/i><\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/span>\n\t\t\t\t\t\t\t\t\t\t\t\tAmount Payable<\/a>\n\t\t\t\t\t<\/div>\n\t\t\t\t\t
  1. Where the disablement is 100%, the Disablement Benefit would be equal to 80% of the monthly earnings of the employee or self-employed.<\/li>
  2. Where the disablement is less than 100%,the DB would be equal to65% of the monthly earnings of the employee or self-employed, multiplied by the percentage (%) of his disablement.<\/li>
  3. The employee or self-employed may elect to receive a lump sum payment in lieu of a monthly Disablement Benefit- (Conditions apply)<\/strong><\/li><\/ol><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t
    \n\t\t\t\t\t
    \n\t\t\t\t\t\t\t\t\t\t\t\t\t\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/i><\/span>\n\t\t\t\t\t\t\t\t<\/i><\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/span>\n\t\t\t\t\t\t\t\t\t\t\t\tDocuments to be produced (in original format + photocopies)<\/a>\n\t\t\t\t\t<\/div>\n\t\t\t\t\t
    1. Birth Certificate.<\/li>
    2. National Identity Card.<\/li>
    3. Medical Certificates\/Evidence; and<\/li>
    4. Bank Name and Bank Account Number.In the case of a joint account, the National Identity Card(s) of Joint account holder(s) should be produced.<\/li><\/ol><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t
